Initial Paperwork

Once we have scheduled the first appointment, I will use a HIPAA compliant email service to deliver the initial paperwork. Please print the forms, complete, and bring with you to our first session. We will go over the forms before beginning any clinical work, and you will have the opportunity to ask questions at that time.  This paperwork must be completed prior to beginning therapy. If it is not completed prior to the beginning of the session, time must be taken from the session for you to complete, meaning that you will be paying me so that you can fill out paperwork.

Although I don’t accept insurance, I do my best to make therapy financially accessible by offering a pretty generous sliding scale.  Please feel free to call with any questions.
